Welcome to our practice

Your initial appointment will consist of a complete dental examination, which includes a review of your medical and dental history, screening for oral cancer, evaluation of the teeth and gums, and inspection of your bite and T.M.J. (jaw joint). You will also go on a tour of your mouth using an intraoral video camera, where you may see yourself in a way you never have before. Drs. Carr may also recommend x-rays, photographs, and study models of your teeth as are necessary to make a thorough diagnosis of your mouth.
When you return for the second, or consultation appointment, you will be presented with a written copy of the findings of the examination and recommendations for treatment. It is important to us that you fully understand your dental needs, the disease processes that occur in the mouth, and why treatment is necessary to maintain excellent oral health. Appointments for cleaning will be scheduled at the consultation.
Please assist us by providing the following information at the time of your examination:
Most recent X-rays if applicable
A list of medications you are presently taking
If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT:
We ask that all patients under 18 must be accompanied by a parent or guardian during the examination visit.
Please alert the office if you have a medical condition that may be of concern.
Let us know if you have a heart murmur or prosthetic joints that require premedication.

Your dental plan is a form of compensation provided by your employer. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the reimbursement.
Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you we will help you process your insurance claims in order for you to receive this maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.
We ask that payment be made at the time of treatment.
At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you a contract we feel morally obliged to honor.
Our treatments are highly individualized as well as our financial agreements with our clients. For your convenience, we offer a variety of payment options. Our treatment coordinator will work out a plan suited to your individual needs.
